In you are interested in submitting a commendation or complaint, please do the following:

2. Complete the form in your web browser, and save a copy of the file to your computer. Submit the form as an attachment via e-mail to firstname.lastname@example.org.
Please note: You will receive e-mail notification from the Anacortes Police Department within two days if you choose to be contacted.
Alternatively, you can fill out the form, print it out, and deliver the completed form to the Records window at the Anacortes Police Department, located at 1218 24th Street, during business hours (Monday through Friday, 8:00 AM to 5:00 PM) so your request may be processed.
